I understand that the $100 application fee is non-refundable. On the next page, I will fill out the mastermind application. If accepted, I will choose one of the following payment plans:

• Two installments of $5k for a total of $10k (due July 15th and August 15th)
• or Four installments of $2,750 for a total of $11k (due July 15th, August 15th, September 15th, and October 15th)
